Care Manager (RN)
HealthCare Support is actively seeking a Care Manager (RN) to fill an opening with a Managed Care Organization in South Carolina.
Daily Responsibilities for Care Manager (RN):
Transition of Care Case Management for Medicaid members, telephonic outreaches to facilities and members. Assist facility staff with discharge planning efforts for members.
Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families.
Shift: Mon- Fri between 8am- 5pm
REMOTE position
Required Qualifications for Care Manager (RN):
Clear and active South Carolina RN license
2+ years of clinical nursing experience, preferably in case management or from hospital setting
Graduate from accredited school of nursing
MS office – Outlook, Word, Excel, Teams
Preferred Qualifications for Care Manager (RN):
BSN
