Job Title: Michigan Choice Adult Waivers Specialist (Remote)
Job Summary
We are seeking a knowledgeable and experienced Michigan Choice Adult Waivers Specialist to join our team remotely. The ideal candidate will have a strong background as an adult care worker and a thorough understanding of Michigan’s Home and Community-Based Services (HCBS) waiver programs. This position requires expertise in Medicaid waiver services, eligibility determination, care coordination, and compliance with Michigan’s Department of Health and Human Services (MDHHS) regulations.
The specialist will work directly with Medicaid beneficiaries, caregivers, providers, and community resources to ensure individuals receive the support and services they need to remain independent in their homes and communities.
Key Responsibilities
Assist individuals in understanding and accessing the Michigan Choice Waiver Program, ensuring they receive appropriate home and community-based services
Conduct eligibility assessments, care planning, and ongoing case management for waiver participants
Work collaboratively with healthcare providers, community organizations, and family caregivers to coordinate services and resources
Maintain compliance with Michigan’s Medicaid waiver program policies and MDHHS regulations
Document all assessments, service plans, and communications in accordance with Medicaid and agency guidelines
Advocate for participants to ensure they receive the necessary support to live independently in the least restrictive environment
Provide guidance on Medicaid waiver program requirements, application processes, and benefit eligibility
Monitor and review participants’ care plans to ensure appropriate service utilization and quality of care
Stay up to date with changes in waiver programs, policies, and regulations affecting services in Michigan
Qualifications
Minimum of two years of experience as an adult care worker, case manager, or social services professional
Extensive knowledge of the Michigan Choice Waiver Program and related Medicaid home and community-based services
Experience working with older adults, individuals with disabilities, or vulnerable populations in a care coordination or advocacy role
Strong understanding of MDHHS regulations, Medicaid eligibility, and HCBS waiver funding guidelines
Excellent communication, problem-solving, and organizational skills
Ability to work independently in a remote environment with minimal supervision
Proficiency in electronic health records, case management systems, and Microsoft Office Suite
Bachelor’s degree in social work, healthcare administration, human services, or a related field preferred. Experience in lieu of degree is acceptable.
