If you are located in the state of California or willing to work within Pacific Standard Time zone, you will have the flexibility to work remotely* as you take on some tough challenges. The work schedule for this opportunity is Thursday – Sunday, 4 days/ 10 hour work shifts within the hours of 7am – 7pm.
Primary Responsibilities:
Assessing barriers to a safe and efficient discharge process, identifying the relevant resources, and finally coordinating and driving an efficient discharge process with both internal and external stakeholders
Prepare and attend team huddle each morning to prepare the discharge census and prioritize patients/discharges/issues/facilities
Collaborate on cases with RN DCM as needed
Prepare the weekend census and potential discharges, being astute & observant to anticipate potential discharges and barrier during the weekends and aim to resolve them ahead of time
Initiate proactive outreaches to facility CM/UM/Discharge staff upon generating or receiving a case referral to manage the discharge, working collaboratively with the facility discharging staff to
assist in resolving any discharge barriers and continuing to engage facility staff until the actual discharge
Collaborate closely with internal and external partners to aggressively drive safe discharges to achieve higher daily discharge volume and overall lower LOS
Continuously look for opportunities to improve and streamline DCM process, to maximize discharge efficacy & efficiency
Cultivate and build relationships, especially where one does not exist or is challenging, with hospitals and their personnel to build a collaborative discharge management relationship
Other tasks as requested by their manager

Required Qualifications:
Current and unrestricted RN license in the state of state of residence
5+ years clinical nursing experience in Discharge Care Management
Ability to work Thursday- Sunday, 4-10 hour schedule in PST time zone

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
