The FUH Coordinator will focus on FUH (follow-up after hospitalization) and meeting our HEDIS goals by assisting facilities to obtain aftercare appointments for our members who have been psychiatrically hospitalized and provide resources to facilities to assist them with discharge planning.
Fully Remote (City, State residency required see above for locations) Schedule Monday-Friday. 40-hour week, an 8-hour shift between the hours of 7:00 am to 6:00 pm based on location.
Travel: Up to 10% travel to visit facilities.
If you are located in Washington you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:

Manage the requests for help with discharge planning from facilities

Work to obtain FUH (follow-up after hospitalization) details for members in Mental Healthcare inpatient admissions

Develop and maintain relationships with facilities to provide assistance in discharge planning

Provide data to Case Management to support our members

Support HEDIS goals around FUH (follow-up after hospitalization)

Required Qualifications:

High School/GED

5+ years of experience in healthcare setting

2+ years of experience in managed care environment (insurance side preferred)

1+ years of MS Office experience to include Word, Excel and Outlook

Preferred Qualifications:

Bachelor’s degree in healthcare related curriculum

Proven Provider/Facility outreach experience

Proven FUH (follow-up after hospitalization) knowledge

Proven HEDIS knowledge

Demonstrated flexibility with work hours when needed

Demonstrated ability to work Independently from home

Proven organizational skills

Proven exceptional time management skills

Proven both Verbal and Written communication skills

Proven detail oriented skills

Proven technology skills (ICUE, LINX, STAR)

Washington Residents Only: The hourly range for this role is $19.47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
