Summary
Lee Health is seeking an LPN to join our Care Transitions Team. This will be located in Fort Myers across from Gulf Coast Medical Center. The schedule will be hybrid with 2 days per week in office and 3 days remote. Hours will be 8:00-4:30pm but with some flexibility. The ideal candidate will have at least 3 years of experience working as an LPN with heavy preference on ambulatory care.
This role requires exceptional interpersonal and communication skills, a passion for nursing, and a desire to utilize his or her clinical experience to assist patients in their transition from hospitalization to home. This position has flexibility and creativity in problem solving techniques to alleviate to barriers to care and demonstrates the ability to work effectively as a member of an interdisciplinary care team while working remotely. Care Transitions Nurses serve as liaisons to empower recently discharged patients to maintain their well-being in the comfort and familiarity of their own home.
LPN Care Transitions Nurse will work in an office setting and will work telephonically with recently discharged patients to prevent health complications, improve patient outcomes, enhance access to quality care, and reduce hospital readmissions. This is accomplished by providing prompt care coordination for patients during transitions between health care settings. The position involves telephonic outreach to patients during the post-acute time frame then working closely with the patient, caregiver, physician, RN care coordinator, care team, and acute care facilities to ensure the patients individual needs are met. Other duties may include performing tasks such as completing health risk assessment for annual wellness visits, documentation and scheduling of transitional care management visits and blood pressure collection and education promoting SMBP. The Licensed Practice Nurse will work closely with the Patient Outreach team collaborating to collect HEDIS and HEDIS like measures for payer and MSSP initiatives.
Requirements
Education: Graduate of an accredited vocational practical school of nursing.
Experience: Minimum of 3 years of clinical experience with emphasis on patient education, chronic disease care; home health, skilled nursing, or medical surgical nursing required. Care coordination experience preferred.
Certification: N/A
License: Licensed as an LPN in the state of Florida.
Other: Proficient computer skills, including Microsoft Office (Word, Excel, and PowerPoint) required. Knowledge of adult learning principles and education methods required. Exceptional communication skills and ability to develop and maintain effective relationships with patient, staff and care team required.
